The performance appraisal is a communication tool designed to support each individual's contribution to the University. The appraisal provides a way to measure skills and accomplishments with reasonable accuracy and uniformity. It provides you with a tool to help identify barriers to top performance. It should help identify areas for professional growth. It should not, however, be considered your only communication tool. Open lines of communication throughout the year keep barriers from rising and help to make effective working relationships.

Each employee is entitled to a thoughtful and careful appraisal. Its success depends on your supervisor's willingness to complete a constructive and objective assessment, and on your willingness to assess your own work, respond to constructive criticism a nd to work with your supervisor to overcome problems that affect your performance.

This is a performance appraisal form designed for self-assessment. As you review your work during the evaluation period, think about your accomplishments, your goals, areas that need improvement and obstacles that may be keeping you from doing your best w ork.
